Outcomes One, Inc. Under Investigation for Data Breach of 149,904 Records

Schubert Jonckheer & Kolbe LLP is investigating a data breach that led to unauthorized access to the sensitive information of 149,904 members of health plans that work with Outcomes One, Inc. (“Outcomes One”), a Florida-based company that provides health plan members with medication therapy management and medication adherence services.

On or around July 1, 2025, an email phishing incident occurred, and an unauthorized actor gained access to an employee’s email account and gained to files and emails containing sensitive data. The data breach notice submitted to the California Office of the Attorney General indicates that Aetna Health Insurance Company is one of the health plans impacted by the breach.

Although the breach occurred in July 2025, Outcomes One did not begin notifying affected individuals until on or around September 23, 2025, which may have violated state and federal laws. The following information may have been compromised in the breach: name, demographic information, medical provider name, health insurance information, and medication information.
